Full Proposal Guidelines

 

Cover Sheet

Full proposals must include a one-page, single-sided, double-spaced cover sheet with the following information:

  1. Organization name, address, telephone & fax numbers and, if applicable, website
  2. A copy of your IRS 501(c)3 determination letter or a letter from your fiscal sponsor
  3. Name, title, email address and telephone number of the contact person
  4. Purpose of your organization (include mission statement)
  5. Brief title of proposed project
  6. Brief one-paragraph summary of the purpose of the requested grant
  7. Amount requested and period of time over which you seek it
  8. Name of Horizon trustee or staff member with whom you discussed your proposal

Project Description

Following the cover sheet, submit a complete project description, on a maximum of FOUR PAGES single-sided, double-spaced. The project description must include the following information:

  • the need and planned response to the stated need
  • expected accomplishments, and in-depth description of how they will be attained
  • who will lead your efforts, including their experience as educators
  • a detailed explanation of how you will document and evaluate the effectiveness of your project

Budget and Financial Information

On separate pages, please include the following information:

  • a detailed project budget showing how the grant funds will be spent
  • an overall organizational budget, if different from project budget
  • your organization’s most recent audited financial statement
  • a list of other sources of grant and gift funding for the past two years
  • whether any challenge matches might apply to this project
  • a list of other foundations and corporations from whom you are soliciting grant funding for this project
  • a list of your organization’s Board of Directors or Trustees
